We all know doctors do not have the amount of time to spend with patients as they would like to have. Home Care should be an extension of the services that you, as a physician, can offer your patients. In depth teaching from a RN or LPN in areas such as: medication management, disease processes, new diagnoses, wound care, and nutrition/diet, is just one area of what home care can help you with. Patients with home care nurses are less likely to make those frequent phone calls to your office with questions. There is no doubt that patients still have to see their physician, but when a patient’s doctor sets them up with Home Care, that is telling that patient that you care enough to take the time to make sure that all of their needs are taken care of. We can be there when you can’t. |

Healthcare is a team activity. Doctors, nurses, hospitals, home care, hospice, nursing homes, assisted living, and rehab facilities all go hand in hand. Use home care as a way to promote your practice. Saying that you support and refer to home care is an attractive feature to the aging population. Knowing that you will do your best to help them stay in their home gives them peace of mind. |

Do you have patients that sometimes you feel are not telling you the whole story? Would you like to have a synopsis of what has been happening with your patients before you walk into the examination room? How impressed would a patient be if you told them how great their blood sugars have been running the past month before they even have the chance to tell you? We can help you with that! Prior to a patient’s appointment with you, we will send you a one page update on your patient that includes; BP ranges, blood sugar ranges, edema, chest pain, respiratory/breath sounds, skin integrity, pain, whether or not they are compliant with their medications, a current list of their medications, if they have had any recent falls, and any other pertinent issues. |

We can work with the doctor to keep patient’s in their own home. Home truly is where your heart belongs. Studies show that home care patients are happier, healthier, and live longer. Home Care is undoubtedly the least expensive form of health care, which helps to reduce national spending on health care and benefits all tax payers. Home care can help minimize recurrent hospitalizations of your high risk patients, such as those with Diabetes, COPD, CHF and mobility problems. But how does home care benefit the physician? Fewer hospitalized patients means less time required making rounds, which means the physician can spend more time in his practice, seeing more patients and increasing your revenue. |
